Insurance
When children are scheduled for well visits, it is the parent's responsibility to verify insurance benefits before the visit. The adult bringing the child into the office must bring valid and current insurance information to each visit.

Referrals
Referrals to specialists require 2 business days. Unless there is a true medical emergency, we will not be able to provide an immediate referral when we are called from a specialist's office.
Copays
Copays are expected at the time of the visit. For your convenience we now have a credit card machine, in addition to accepting cash and checks. There will be a $5 billing charge for copays not paid at the time of the visit.
Forms
Our schools, camps, and childcare facilities are requiring more and more forms to be completed by the doctors. These forms should be brought to the office at the time of the well visit. If they are brought at other times, there will be a $10 fee. All forms require 2 business days for completion.
Copies
There will be no charge for copies of immunization records and last physical exams. A $10 fee will be charged for duplication of an entire chart.
Returned checks
Checks returned from the bank will incur a $15 fee.
Billing
Payment balances received from our billing department are due immediately upon receipt. Billing questions can be answered by our billing staff at (914) 771-7335.
Late policy
Late arrivals force us to inconvenience other parents who were on time. Patients arriving 15 or more minutes late for a well visit will be asked to reschedule. Late patients who are ill will be squeezed in whenever there is space, but may be required to wait.
Cancellations
Last minute cancellations and missed appointments represent a significant cost to the practice. Missed well visits and well visits cancelled less than 24 hours before the appointment will incur a cost of $25.
